Distance Education Certificate Request Form
After you submit the request for a Certificate, your transcript and application will be reviewed to ensure that you have met the requirements of the certificate you applied for. If you have not applied to a Soil and Water Science Certificate Program, you will not receive a certificate.
I affirm that I have completed the courses listed in my Certificate Program application and have maintained a 3.0 GPA on all required and elective coursework for the certificate.
